Most institutions can produce a grade sheet in minutes and an attainment report in three weeks, because the marks live in a gradebook and the outcomes live in a spreadsheet built the month before the audit. The Assessment & Outcomes lifecycle connects patterns and weights, outcome mapping, exam administration, grade entry, moderation, approvals, publishing and attainment on one governed model, so every reported number can be traced back to the item that produced it.
The problem
Marks are recorded all year. Learning is asserted in one week.
Evidence is produced all term, in the right tools, by people doing their jobs properly. It is simply never kept in a form anyone can verify, so the attainment figure in the self-assessment report gets calculated by hand a few weeks before the panel arrives.
Nobody is being dishonest. The institution just cannot show its working, and neither can the next coordinator who inherits the course.

How to think about assessment.
One course, one term, run two ways. The teaching is identical: what changes is whether the attainment number at the end can be reproduced by someone who was not in the room.
Nothing publishes until moderation and approval are complete, and every computed figure stores its inputs, weights, method and policy version: which outcomes were measured, by which assessments, marked by whom, moderated how, and can you show the same number again in front of us.

An attainment figure a model produced but nobody can explain is worse than no figure at all. Computation here is deterministic: same inputs, same method, same answer, every time. AI suggests mappings, flags anomalies and drafts improvement actions, and a person approves every one of them. The structure is the lifecycle itself; the model rides on top.
Method 2 · reliability MED
Mapping suggestions with coverage checks
When an assessment is created, the system proposes the outcomes its items most likely measure and flags any course outcome left below the policy minimum. Faculty accept, adjust or reject; the mapping is theirs, the checking is automatic.
Grade distribution and variance alerts
Extreme skew, bimodal spreads, outliers and large differences between sections of the same course are surfaced to the coordinator before publication, with the cohort and the marker attached so the review has somewhere to start.
Integrity checks before the gate
Missing grade entries against the enrolled roster, stale imports from the learning environment, outcomes measured by a single assessment and plans that breach policy are all caught while there is still time to correct them.
Improvement actions from prior cycles
Where an outcome falls short, the recommendation draws on what changed the number last time this cohort or a comparable one was taught: a rubric revision, a reweighting, an extra formative check. The committee decides; the action is tracked to closure.

What is the Assessment & Outcomes lifecycle?
It governs evaluation: assessment patterns and weights, outcome mapping, exam administration, grade entry, moderation, approvals, result publishing and attainment computation.
In Creatrix Campus this is one of the institutional lifecycles in the Academic Operating System, sitting between academic delivery and the official student record.
How is CO and PLO attainment computed?
Deterministically, from mapped assessment items, using the method your policy selects: average, percentage above threshold, or weighted by assessment weight.
Every computed record stores its inputs, weights, thresholds, method and policy snapshot, and carries a reliability flag of low, medium or high. Same inputs, same answer, in front of a panel or a year later.
We deliver assessments in Moodle or Canvas. Does this replace the LMS?
No. The lifecycle runs in three modes: LMS primary, where structure and marks are imported and governance runs here; Creatrix primary, where assessments are delivered and graded natively; and hybrid, chosen per offering.
Grades originate at the delivery point. Once moderation and approvals are applied, the moderated grade becomes the governed record for publishing and reporting, which is what stops the gradebook and the official result from disagreeing.
How is moderation enforced before results are published?
Publishing is blocked until moderation, where policy requires it, and the approval chain are complete.
Sampling strategy, sample size, statistics before and after adjustment, the adjustment log, the moderator and the approver are all recorded, and the same separation applies everywhere: the person who grades is not the person who authorises release.
Can grades change after publication?
Only through the amendment workflow. Publication, approval and finalisation each lock a defined scope, and a locked result cannot be quietly edited.
A change request is reviewed by the registrar and approved by the head of department. The old mark is preserved, a new version is created, and the reason, approvers and timestamps stay on the record.
Does it support competency-based education and CBME?
Yes, without a separate module. Competencies, milestones and EPAs are governed definitions; this lifecycle owns attempt records, workplace-based observations, multi-assessor scoring, entrustment decisions and mastery computation.
Mastery receipts are published to student records, which holds the official competency transcript, and remediation plans are tracked to completion.
How does an accreditor verify a reported number?
By opening the trace. Each attainment record links back to the assessment items, the mapping weights, the grade entries, the moderation log and the policy version in force at the time.
